Risk factors for severe withdrawal, including seizures and delirium tremens, include a longer history of heavy drinking, a prior history of severe withdrawal or delirium tremens, concurrent acute illness or injury, and older age. A prior episode of severe withdrawal is one of the strongest predictors of a severe episode in the future, reflecting a phenomenon called kindling, where each successive withdrawal episode can become progressively more severe.

Comorbidity with alcohol use disorder is, by definition, central to this condition, and withdrawal risk and severity should be carefully assessed as part of any treatment planning for AUD, particularly before someone attempts to stop drinking.

What causes it

Alcohol withdrawal results from the brain’s adaptation to chronic alcohol exposure, and the syndrome reflects what happens when that adaptation is suddenly left unsupported by alcohol’s continued presence.

How it's diagnosed

Alcohol Withdrawal is diagnosed based on cessation or reduction of heavy, prolonged alcohol use, with two or more of the following developing within several hours to a few days: autonomic hyperactivity (sweating, pulse over 100), increased hand tremor, insomnia, nausea or vomiting, transient visual/tactile/auditory hallucinations or illusions, psychomotor agitation, anxiety, and grand mal seizures. The symptoms cause significant distress or impairment and aren’t attributable to another medical condition or substance. A specifier indicates with perceptual disturbances when hallucinations or illusions occur with intact reality testing (distinct from delirium tremens).

Differential diagnosis is critically important given the medical seriousness involved. Delirium tremens represents a more severe presentation than uncomplicated withdrawal, involving significant disorientation and loss of awareness that hallucinations aren’t real, requiring its own urgent recognition and treatment. Other causes of similar symptoms, including other substance withdrawal, infection, electrolyte abnormalities, and head injury, should be considered, particularly when the clinical picture doesn’t fit cleanly, given how much overlap can exist between these presentations and alcohol withdrawal, especially delirium tremens.

Treatment

Alcohol withdrawal treatment depends on assessed severity, but the central, overarching principle is that significant, sustained drinking should not be stopped without medical guidance.

Benzodiazepines
Benzodiazepines are the cornerstone of medical withdrawal management, working through the same GABA system alcohol affects, allowing for a controlled, gradually tapered reduction that prevents the dangerous rebound hyperexcitability of unmanaged withdrawal. Supportive care
Thiamine supplementation is routinely given to prevent a serious neurological complication (Wernicke’s encephalopathy) associated with alcohol use, alongside attention to hydration, nutrition, and electrolyte balance throughout the withdrawal process.

Transition to ongoing treatment
Once acute withdrawal has been safely managed, transitioning to ongoing alcohol use disorder treatment, addressing the underlying pattern that led to physical dependence, is an essential next step to support lasting recovery and reduce the risk of repeated, potentially worsening withdrawal episodes in the future.

Self-care & coping

Do not attempt to stop heavy, sustained drinking alone, without medical guidance. This is the single most important piece of information in this entire entry. Medically supervised withdrawal management exists specifically because unsupervised withdrawal can be genuinely dangerous.

The outlook for alcohol withdrawal with appropriate medical management is generally very good; the large majority of people, even those with significant withdrawal risk, are safely and successfully managed through the acute period with modern, structured treatment approaches.

Without medical management, particularly for those with significant physical dependence, the risk of serious complications, seizures and delirium tremens, is meaningfully higher, and delirium tremens carries a real, though now considerably reduced with modern treatment, mortality risk.

The kindling phenomenon means that each successive unmanaged or poorly managed withdrawal episode tends to increase the risk of a more severe episode in the future, which is part of why proper management of even a first or seemingly mild episode matters considerably for long-term safety.

Successfully managing acute withdrawal is just the first step; the underlying alcohol use disorder requires its own ongoing treatment to support lasting recovery and prevent the cycle of repeated, escalating withdrawal episodes.
